Your family must reside at least 50 miles from the Ralph H. Johnson VA Medical Center to qualify.
Veteran Receiving VA Care
The Veteran must be actively receiving qualifying treatment through the Ralph H. Johnson VA Health Care System.
VA Team Referral Required
A social worker, case manager, or VA provider must submit the referral on your behalf.
Independent Living Environment
Guests must be able to stay safely in an independent living environment without medical supervision.
